In the Event of a claim

  • Immediately contact the proper authorities by dialing 911 (ambulance, police, fire).
  • Make all possible effort to protect individuals from personal injury.
  • Take necessary measures to protect your property from further damage.
  • Comply with the request of proper authorities. Do not speculate, make blunt admissions or take unnecessary blame.

  2. WHEN, HOW and WHERE did the event occur?
  3. What exactly happened?
  4. Were there any witnesses or injured parties?
  5. Record the names, addresses, and phone numbers of involved parties.

24 Hour Claim Reporting

During business hours, please report your claim directly to Mountain State Insurance Agency. In the event your claim occurs outside our normal business hours, and requires IMMEDIATE attention, contact the appropriate company below, however, please notify our office the following business day.
